PCOS is diagnosed when 2 of 3 criteria are met (Rotterdam criteria): irregular/absent periods, clinical/biochemical signs of excess androgens (acne, body hair, hair thinning), and polycystic ovaries on ultrasound. It's the most common cause of anovulatory infertility. 70%+ of PCOS women have insulin resistance. Management includes lifestyle changes (diet, exercise), metformin, birth control pills (for cycle regulation), and fertility medications (Clomid, letrozole) when trying to conceive.
