The size and shape of sperm cells, assessed as part of a semen analysis. Normal morphology (strict Kruger criteria) is ≥4% normally shaped sperm.

Sperm morphology assesses the head, midpiece, and tail of sperm cells. Abnormal morphology (teratozoospermia) may include misshapen heads, double heads/tails, or coiled tails. Even with low morphology percentages, natural conception is possible — it just may take longer. Morphology below 4% (strict criteria) is associated with lower IUI success rates, making IVF with ICSI (where a single normal sperm is injected into the egg) a better option. Lifestyle improvements (diet, stopping smoking, reducing alcohol) can improve morphology over 2-3 months.
