About 20% of women regularly experience mittelschmerz. The pain can range from a dull ache to a sharp twinge and typically lasts minutes to hours (rarely more than 1-2 days). It may alternate sides each month depending on which ovary ovulates. While it's a useful natural indicator of ovulation, it can occur slightly before, during, or after the egg is actually released, so it shouldn't be relied upon as the sole fertility indicator.
